The ActiveDogs Registered Service Dog 3 Key Tag Cards provide a convenient and durable identification solution for service dog handlers. This unique 3-part identification card contains your service dog's information and can be separated into three individual key tag IDs for easy access and everyday use.
Each ID card is made from heavy-duty 30 mil plastic, the same material used for many state-issued identification cards. A professional resin thermal encoding process creates a polished, durable finish, while lamination on both sides provides added protection for everyday use.
The 3-part key tag design allows you to break the card into three smaller identification tags that can be carried separately or attached to different items for quick access when needed.
Included with your purchase is a FREE entry into the ActiveDogs online registry database, allowing your service dog ID to be validated from a smartphone, tablet, or computer. You will also receive a FREE digital copy of your completed ID card via email within 1 business day after production.
To create your custom ID card, simply submit your dog's photo and information using the ID Information Form. Headshot photos work best, although we will do our best to edit other photo styles when possible.
On the back of the IDs: Service Dog IDs include ADA (Americans with Disabilities Act) information outlining public access rights for service dog handlers.

Service Dogs must be individually trained to perform tasks for a person with a disability. Misrepresenting a pet as a Service Dog may violate state or local laws and result in penalties.
Service Dog: is defined as a dog that has been individually trained to do work or perform tasks for an individual with a disability. The task(s) performed by the dog must be directly related to the person's disability.
Therapy Dog: is defined as a trained therapeutic dog that provides psychological or physiological therapy to individuals other than their handler. These dogs do NOT have ADA public access rights and must have permission from an establishment to enter.
