The ActiveDogs Horizontal Identification Card provides a professional and durable identification solution for service dogs, therapy dogs, search and rescue dogs, and other working dogs. This high-quality horizontal ID card displays your dog's photo and important information in an easy-to-read format while offering convenient identification in public, training, and working environments.
Each ID card is made from heavy-duty 30 mil plastic and printed using a professional resin thermal encoding process for a polished, durable finish. The card is laminated on both sides for added protection and long-lasting use.
Included with your purchase is a FREE entry into the ActiveDogs online registry database, allowing your dog's ID to be validated from a smartphone, tablet, or computer. You will also receive a FREE digital copy of your completed ID card via email within 1 business day after production.
If ordered as a Service Dog ID, ADA (Americans with Disabilities Act) information will be printed on the back of the card outlining public access rights for service dog handlers. Therapy Dog and Search & Rescue IDs include information specific to the work these dogs perform.
The slot at the top of the card allows for multiple attachment options. Attach it directly to one of our capes or padded harness vests, use an optional clip or split ring to attach it to a bag, coat, collar, or gear, or pair it with a lanyard for easy display.
To create your custom ID card, simply submit your dog's photo and information through the ID Information Form. Headshot photos work best, although we will do our best to edit other photo styles when needed.

Please Note: This ID card and registry are not required by the ADA and are offered as an optional convenience item. Service Dogs must be individually trained to perform tasks for a person with a disability. Service Dogs are protected under federal ADA law and are generally permitted access to public places and accommodations. Misrepresenting a pet as a Service Dog may violate state or local laws and result in penalties.
Service Dog: is defined as a dog that has been individually trained to do work or perform tasks for an individual with a disability. The task(s) performed by the dog must be directly related to the person's disability.
Therapy Dog: is defined as a trained therapeutic dog that provides psychological or physiological therapy to individuals other than their handler. These dogs do NOT have ADA public access rights and must have permission from an establishment to enter.
